Dr. Sandra Wegner

Dr. Wegner completed her medical degree at Dalhousie University in Halifax, N.S. and her family practice residency in Beverly, MA. She has practiced as a family physician in Saskatoon since 1997. She is passionate about creating a medical home model of holistic care at LHC. Dr. Wegner also consults in menopause issues at the Women’s Mid-Life Health Centre at the Saskatoon City Hospital. Sandra and her husband Leon are blessed with six children and a daughter-in-law. Sandra enjoys horses, gardening, photography, jogging, hiking, and camping.

Dr. Chelsea Schemenauer

Dr. Schemenauer grew up in rural Saskatchewan. She earned her Bachelor of Science in Physiology and her Medical Degree from the University of Saskatchewan. In Prince Albert, she completed her Rural Family Medicine Residency and worked for several years as a family physician, before coming back to Saskatoon. She enjoys reading, running, photography, and travelling with her family.

Dr. Tasnim Gafoor

Dr. Tasnim Gafoor completed her medical degree at the Nelson Mandela Medical School in South Africa. She has worked as a family physician in both community and hospital settings, serving both rural and urban centres in Saskatchewan. In 2012 she completed specialized training in cosmetic procedures at the Vancouver skin and laser Centre. She thereafter trained in the use of Botox for chronic migraine headaches with the Department of Neurology at the University of Calgary, Alberta.

In 2015 she completed her fellowship in Cosmetic and Anti-aging Medicine through the American Academy of Anti-aging Medicine, in affiliation with the University of South Florida.
Dr. Gafoor has a special interest in skin and skin conditions, as well as female and child medicine.

Dr. Nassrein Hussein

Dr. Nassrein Hussein grew up in Edmonton where she completed her medical school and internal medicine training at the University of Alberta. She then completed fellowship training in Endocrinology and Metabolism at McGill University. Dr. Hussein is specialized in the diagnosis and treatment of a variety of endocrine conditions including diabetes, obesity, osteoporosis, polycystic ovarian syndrome, thyroid disease, adrenal gland disorders, gonadal disease, and diseases of the pituitary gland.

Dr. Hussein has two young children and in her free time enjoys traveling and spending time with friends and family.

Dr. Alhussein Alfaour

Dr. Alhussein Alfaour has joined our practice from Wadena.  He is an asset to our team and is currently accepting new patients!  You can find Dr. Alfaour working several evenings and weekends as well in order to better serve your needs.  Please note that Dr. Alfaour DOES NOT prescribe medical marijuana, nor does he refill narcotic prescriptions.

Dr. Shams Fakhir

Dr. Shams Fakhir brings a wealth of experience in general pediatrics. After more than 12 years of busy general pediatric practice in Swift Current, Saskatchewan, he has recently relocated to Saskatoon. He is an Assistant Professor of Pediatrics in the College of Medicine, University of Saskatchewan. Dr. Fakhir is a strong believer in providing a medical home to his patients. He is the founding member of Prairie Wellness Initiative-a program to improve the overall health and wellbeing of children in Southwest Saskatchewan. He was awarded quality champion award from Health Quality Council for his achievements in 2013. Dr. Fakhir is committed to providing a full array of general pediatric services including well baby examinations and health supervision. His areas of special interest include: pediatric and adolescent mental health, weight management, asthma and allergies, and urgent care. Dr. Fakhir is the father of two boys.


Dr. Ruben Rajakumar

Dr. Ruben Rajakumar has been a medical practitioner since 1971.  A graduate of the University of Peradeniya, Sri Lanka, he began his medical career in general practice and internal medicine in Sri Lanka.

After completing residency training in internal medicine and a sub-specialty fellowship in cardiology at the University of Saskatchewan, his family relocated in 1991 to Saskatoon.

In addition to his substantial clinical practice and broad clinical knowledge, Dr.Rajakumar has a sustained interest in applied clinical research and knowledge translation.  He has distinguished himself as an expert clinician and a clinical trialist.  Having been both a principle investigator and co-investigator of many clinical trials, conducted in North America and other parts of the world over the past fifteen years, he is well known to the pharmaceutical industry.

Currently he is promoting health care innovations to provincial, federal governments, academic institutions and indigenous leaders to improve northern and rural health services in Saskatchewan and Ontario.
